Consent

We need informed consent before taking a patient on for care and before starting treatment. YOU are responsible for ensuring a patient you treat has consented to the procedures you are delivering.

Remember consent is only valid if you have explained the risks and benefits of any care so be sure to do this, don’t just rely on a patient signing a form. This protects you and gives the patient a chance to opt out of anything they are not happy with.

We use an electronic consent sent to a patients phone they sign with their finger:

Consent before the first appointment (Visit one)- Please complete the attached form giving us consent to start your care: https://bit.ly/3nSYZcB Back Active NHS

Consent before starting treatment - Please complete his form to give us consent to start treatment: https://bit.ly/2UOkDBY Back Active

Use the SMS send function in a patients presentation in Care Response to send the consent request. This is simple and keeps a record that it was sent.

When a patient returns an electronic consent form one of the admin team will load it into Care Response - this may take a day or two. I suggest also noting in the records if a patient sends a consent whilst in the treatment room incase there is a problem with the technology.

If for any reason a patient can not complete an electronic consent have them complete a paper one and you give this to reception who will scan and load onto Care Response.
